Submit Proof of Vaccination

Due to the State of Illinois Executive Order issued on August 26, 2021, Illinois colleges and universities must gather proof of COVID-19 vaccination for registered students taking in-person courses or activities. You may use this form to submit your proof of vaccination for COVID-19 to Highland Community College. You may also submit documentation to a staff member in enrollment services during business hours (Student/Conference Center on the second floor of Building H, or to Student Services on the first floor of Building H). All records remain confidential.

If you are not vaccinated, weekly testing will be required in the near future. More screening test information will be forthcoming.

Highland will accept proof of COVID-19 vaccination at any time you begin and complete the vaccination. If you have already received one or two doses of the vaccine, please upload your proof of vaccination by September 15, 2021.

* Proof of vaccination includes a vaccination card, physician's record or letter, or state immunization record or documentation.
* The image should clearly show your name, the type of vaccine received, and the date(s) of vaccination.
